What are your office hours?

Our office hours are as follows:

Monday through Friday: 8:00 am – 4:45 pm

Saturday: 8:00 am – 12:45 pm

Sunday: Closed

We are closed on the first Saturday of each month.

Appointments with a physician are available beginning at 8:45 am Monday through Friday and 8:15 am on Saturdays.

Which insurance plans do you accept?

We accept most insurance providers and offer payment options for uninsured patients. However, if you are a first-time patient or your insurance provider has changed, please call your provider to verify. If you are not covered under a health insurance plan, please call us for a consultation at (219) 659-0333.

The typical wait time is 15 to 30 minutes.

What can I expect at my visit?

Whether you are an existing or first-time patient, we will strive to make your visit as productive and comfortable as possible.

If this will be your first time visiting our clinic, below are a few tips to help make your visit a success. Please call if you have any questions.

  • Arrive approximately 15 minutes prior to your appointment to allow time for filling out paperwork.
  • Bring your health insurance information.
  • Write down any questions or concerns you may have so that you can discuss them with your physician.
  • If you are transferring from another primary physician’s care, bring your medical records.

If you are an existing patient, there is no need to do anything in advance. Please arrive at your scheduled time and sign in at the front desk. If you have any questions regarding your appointment, please call us at (219) 659-0333.

What Does a Primary Care Physician Do?

How Often Do I Need to Visit My Primary Care Physician?

Visiting your primary care physician is critical to maintaining your health. We can order diagnostic tests and use state-of-the-art medical technology to identify signs of disease before they progress. We can help you develop an effective care plan to maximize your current well-being and safeguard your health in the future. Health insurance plans often consider annual physical exams as preventative care, so plan on visiting our office at least once a year for a general checkup. You may need more frequent care depending on your needs.
