What to Bring to Your First Primary Care Appointment

Whether you just moved to the area or you’re working on prioritizing your health more, starting a relationship with a new primary care provider is one of the best steps you can take for your long-term health. Walking in to that first appointment unprepared, though, can slow things down and leave less time for the conversations that actually matter.

At OccuMed Primary Care in Whiting, our goal is to make every visit as productive and comfortable as possible for our patients. We know that there are a few things you can do to help meet that goal before you even step into our office! Gathering information before you come can be especially helpful.

Here’s a quick summary of what to bring to your first appointment:

  • Current medications and supplements
  • Previous medical records
  • Insurance information
  • A list of questions or concerns

Let’s look a little closer at why each of these is so important.

A List of Your Medications and Supplements

Bring a complete, up-to-date list of everything you take: prescriptions, vitamins, supplements, and over-the-counter medications. Dosages matter too, so be as specific as possible.

Why do you need to share this? Certain medicinal combinations can interact in ways that aren’t immediately obvious. Your provider needs the full picture of what you’re already taking to make safe, informed recommendations going forward.

Your Previous Medical Records

It can be easy to underestimate how useful past records are to your new provider. A summary of previous diagnoses, surgeries, hospitalizations, and lab results gives important context for your current health, especially for managing chronic conditions like diabetes, high blood pressure, or asthma.

Try to gather:

  • Recent lab results or imaging reports
  • A list of past surgeries or major illnesses
  • Vaccination records, if available
  • Any specialist notes or referral letters

If you can’t find everything in time, don’t stress. Bring what you have, and your care team can help fill in the gaps.

Your Insurance Information

Sorting out insurance at the front desk takes time that could be spent with your provider. Bring your insurance card, a photo ID, and any referral or authorization forms required by your plan so check-in goes smoothly.

OccuMed Primary Care accepts most major insurance plans, including Medicare and certain Medicaid plans. If you’re unsure whether your plan is accepted, you can always check with a quick call before your appointment.

A List of Questions and Concerns

This one gets overlooked often. After preparing for your appointment over multiple days, it can be all too easy to forget what you wanted to ask once you’re in the exam room. Writing your questions down ahead of time helps you get the most out of every appointment.

Consider including:

  • Symptoms you’ve noticed recently, even if they seem minor
  • Family health history you think may be relevant
  • Lifestyle questions (diet, sleep, exercise)
  • Any screenings or lab work you may be due for

No concern is too small. Whether it’s a symptom you’ve been putting off or a question about preventative screenings, your provider is there to help.

Make the Most of Your First Visit

Your first appointment is more than just a checkup: it’s the foundation of an ongoing relationship with your care team. Coming prepared to your first appointment helps your provider understand your full health picture from day one, so they can build a personalized plan that works for you.
